Driver Application:

When applying as a new driver, you are required to fill out all personal information. To fill out the application, you will need to know the following:

Social Security Number

Home address history for the past 3 years

Current driver's License number and driver's License history for
the past 3 years

Employment history up to 10 years.

Citizenship status form

History of traffic accidents, violations, and / or convictions from the last 3 years (including DUI or reckless driving convictions and License suspensions).

Military history (if applicable)

You are also required to be at least 23 with a valid CDL.

A minimum 6-month experience driving Class A preferred, but no worries.

***We will train you.***

Drug tests are also required. Drivers must pay out of pocket for the test. Once your pay begins, you will be reimbursed for the test.

Required Work Documents:

All drivers are required to fill out trip information on envelopes provided.

Information includes:

Your name

Trip/load #

Truck and trailer number

Pick-up information with destination information

Delivery information with destination information

And lastly, Fuel information with receipts

Work Documents Policy:

Drivers who do not properly fill out this envelope can and will be charged $100 for every load envelope not properly filled in.

Drivers who do not turn in a BOL will be charged $200. As nobody can be paid without this and we would be responsible for recovering these documents from locations already visited manually.

During your time as a driver, even through training, you are required to use an ELD. (Electronic log device)

This device helps keep track of your driving on the road as well as information for the company and DOT. You will be provided documents explaining fully how HOS(hours of service) work and how to log your time

ELD:

As you train, you will be provided your own login into the Motive application that will be for you to use as a driver.
What is an ELD:

An electronic logging device (ELD) is a device or technology that automatically records a commercial motor vehicle (CMV) drivers driving time and aids in the accurate recording of the drivers hours of service, replacing the need for paper records of duty status (logs).

Unlike logging software that may run on a standalone laptop, smartphone, or tablet, an ELD system has a tethered component, meaning a device — sometimes called a “black box” — attached to the vehicles electronic control module (ECM) to automatically gather engine-use data.

The overall system may include a smartphone or tablet that receives data from the black box, displays it for enforcement (via digital display or printout), and forwards it to the motor carrier for auditing and storage. The ELD must also be capable of transmitting log data electronically to an enforcement official for auditing purposes.

***Unplugging the device does show up on your ELD. Unplugging

the device is against DOT compliance laws and can result in fees

and loss of CDL according to compliance.***

Driver Disqualification:

Drivers under company policy understand that training is unpaid. Termination or vacating the position before or after training will result in 100% non-payment. You are required to give 2 weeks notice prior to vacating (leaving) your position.

***Warning: Drivers who abandon a truck/trailer will result in the


driver paying recovery fees.The record will also be reported via


DAC***




DAC Report:



• DAC reports are basically detailed accounting of the time spent by a truck driver making deliveries. The DAC report provides information on the period of service, equipment operated, loads hauled, driver status and experience, reason for leaving, rehire eligibility, number of accidents with accident detail, drug and alcohol histories including pre-employment test results, and truck driving school performance records.
